Event Publicly Available Information:
Effective on December 31, 2015, Triad Healthcare Corporation merged with and into Triad Healthcare, LLC. Following the merger, the legal existence of Triad Healthcare Corporation ceased. As per the supplemental indentures dated December 31, 2015, all the obligations guaranteed by Triad Healthcare Corporation are assumed and guaranteed by Triad Healthcare, LLC. Since Triad Healthcare, LLC assumed all of the obligations of Triad Healthcare Corporation, Triad Healthcare, LLC should be the Universal Successor as defined in Section 2.2(a)(vii) of the 2014 ISDA Credit Definitions, to Triad Healthcare Corporation.
